On Wednesday 15 May 2002, at about 0918, ZK-ZSP, a Partenavia P68B, was on a scheduled flight from Gisborne to Napier, when its right engine lost power because of fuel starvation. The aeroplane was 5 km from Wairoa, at 5000 feet and in cloud, at the time. On board were 4 passengers and the pilot.
Incident date: Publish date:12 tourists were flown in two AS350 Squirrel helicopters from Queenstown to a snowfield near Mount Tyndall. One of the helicopters collided with a parked helicopter while landing. All on board were injured, the pilot seriously. The Commission was unable to exclude the possibilities that the pilot misjudged the approach. There was no evidence to suggest that the pilot had a medical condition that contributed to the accident, but that possibility could not be excluded.
Incident date: Publish date:On Saturday 14 April 2007 at about 0300, the tug Nautilus III was towing the barge Kimihia, which was loaded with a cargo of rock, through the Wellington Harbour entrance channel when the barge unexpectedly capsized and deposited its cargo onto the seabed of the main entrance channel to the port. The master and crew of the tug, after advising Wellington Harbour Radio of the accident, towed the barge in its inverted state to Miramar wharf where salvage experts were able to right the barge on the 18 April 2007. Incident date: Publish date:On 13 November 1993 weather conditions were deteriorating rapidly at the snow landing site, and the pilot was hover-taxiing ZK-HGO to return to his original landing position when the helicopter became involved in cloud. The pilot landed immediately but the helicopter was drifting sideways at touchdown and rolled over.

Incident date: Publish date:The Commission is investigating a collision between a northbound freight train and a heavy road vehicle at the Elizabeth Street level crossing in Waikanae at 0620 on 15 October 2025. The reported circumstances were that a large articulated truck (Titan Mack tractor unit) hauling a low loader with an earthmoving vehicle on board) had become stranded on the level crossing while attempting to cross it. The approach of a freight train activated protection barriers, which came down on the tractor unit. The train then collided with the tractor unit.

Incident date: Publish date:Distraction by cellphone and excessive speed within a speed restricted area were the major factors contributing to the fatal collision on 20 June 2008 in Waikawa Bay, Queen Charlotte Sound, of the 6 metre aluminium workboat Shikari with the stern of the moored Flightless, a 27 metre private vessel. The Shikari’s skipper and one passenger died, 3 passengers received serious injuries, and the remaining passenger suffered minor injury. Nobody was aboard the Flightless.
Incident date: Publish date:On Sunday 4 July 2004 at about 1517, while on passage from Chatham Islands to Napier in heavy weather, the coastal cargo vessel "Southern Tiare" lost its rudder below the rudder head. The master was able to manoeuvre the ship, using the bow thruster to maintain the ship’s head into the sea, until a tug arrived to take the ship under tow to Napier, where it berthed without further incident. Incident date: Publish date:On Friday 31 March 2006 at about 0520, the locomotive engineer of empty passenger Train 4306 noticed the air brake system operating below normal performance while driving the train between Westfield and Otahuhu at the start of operations for the day. After the train had reached Otahuhu, the locomotive engineer examined the brake system and decided to return to Westfield rather than continue passenger operations. There was no damage or injuries.
