On Saturday 5 July 2003 at about 2100, the passenger freight ferry "Aratere" collided bow first with the starboard side of a fishing vessel moored at Aotea Quay in Wellington Harbour. The fishing vessel, "San Domenico", and the quay suffered extensive damage. The safety issues identified included: · the undertaking of safety critical tasks while suffering from the effects of chronic sleep loss · the adequacy of provision of medical data concerning sleep disorders in the Maritime Rules

On Friday 4 July, at 0632 the container ship "Bunga Teratai 4" and the fishing vessel "Mako" collided in Tasman Bay to the west of D'Urville Island. Neither vessel was damaged to the extent that its seaworthiness was compromised. Had the point of contact been at the bow of the ship instead of the stern, the fishing vessel would probably have been capsized and sunk with the likelihood of serious injury or death of its 4 crew. On Friday 27 June 2003, Tranz Metro Train 3347 was an Auckland to Papakura diesel multiple unit passenger train. At about 0915, a driveshaft on passenger car ADL 810 failed as the train approached Purewa Tunnel, between Meadowbank and Glen Innes stations. The DMU driver stopped the train about 200 m short of the tunnel. The free end of the failed driveshaft was not sufficiently restrained and it punctured the floor of the passenger compartment. The 3 passengers travelling in ADL 810 were not injured. The safety issues identified included:

On Thursday 16 January 2003, at about 1743, the passenger ferry "Harbour Cat" was proceeding from Birkenhead to Auckland with 3 crew and 2 passengers on board when a fire was discovered in the starboard engine space. The crew extinguished the fire and the ferry continued to the Auckland ferry terminal on its port engine alone. One of the crew suffered from slight smoke inhalation. Safety issues identified included: * access to engine rooms on passenger ferries * adequacy of procedure and training of ships’ crew in tackling engine space fires.

On Friday 6 June 2003, Air Adventures New Zealand Limited Piper PA 31-350 Navajo Chieftain aeroplane ZK-NCA, was on an air transport charter flight from Palmerston North to Christchurch with one pilot and 9 passengers. At 1907 it was on an instrument approach to Christchurch Aerodrome at night in instrument meteorological conditions when it descended below minimum altitude, in a position where reduced visibility prevented runway or approach lights from being seen, to collide with trees and terrain 1.2 nm short of the runway.

On Thursday 15 May 2003, at about 1240 hours, bi-parting doors on Train 3247, a Tranz Metro Auckland to Papakura diesel multiple unit passenger service, closed on an alighting passenger at Glen Innes station and the train departed with the passenger trapped in the door. The driver heard the trapped passenger’s screams and stopped the train within about 15 m from where the train departed. The passenger suffered minor injuries. The safety issues identified were: · the door closure process · the inspection of safety critical door components.

At about 0400 on Monday 5 May 2003, while steaming towards fishing grounds to the west of Fiji, the fishing vessel "Solander Kariqa" suffered a fire in the domestic refrigeration space resulting from maintenance involving oxy-acetylene cutting. The crew fought and contained the fire, which was not totally extinguished until about 2000 that evening. There were no injuries to the crew, but had they been unable to extinguish the fire with the limited fire fighting equipment available the vessel may have been lost with resulting injuries or fatalities. On Wednesday 16 April 2003, at about 1820, the motor tanker "Capella Voyager" grounded while entering Whangarei Harbour to berth at Marsden Point Oil terminal. The hull in the area of the forepeak tank was set up, creased and ruptured, water flooded into the forepeak tank. The "Capella Voyager" subsequently berthed safely at the oil terminal where the cargo was discharged. Temporary repairs were effected and the forepeak pumped out at Marsden Point before the ship sailed for Singapore to complete permanent repairs. There was no discharge of oil into the environment.
