Aviation AO-2003-006

On Friday 3 October 2003 at 2126, Convair 580 ZK-KFU was on a scheduled night freight flight from Christchurch to Palmerston North, when it was observed on radar to enter a tightening left turn and disappear. Attempts to contact the aircraft were unsuccessful and a search for the aircraft was started. The aircraft had impacted the sea about 10 km north of Paraparaumu about vertically and at high speed. The crew of 2 was killed on impact.

On 16 August 2003, at about 1420, as the container ship "Spirit of Enterprise" crossed the Manukau Bar, it encountered several large swells, causing it to pitch heavily. The ship's rudder struck the seabed with sufficient force to fracture the rudderstock causing the loss of the rudder, thus disabling the ship. Safety issues identified were: · insufficient "real time" environmental information · pre-accident cracking of the rudderstock · possible pre-damage to the rudderstock from a previous grounding and the jamming of the flap actuating mechanism

On Saturday 9 August 2003, at about 0135, eastbound express freight Train 337 derailed about 7 m before the west portal of Kaimai Tunnel, between Hemopo and Whatakao. Five empty wagons and one loaded wagon derailed at a failed rail joint. There were no injuries. Safety issues identified included: • the suitability of the standards for rail end mismatch and deformity at joints • the security of a fishplated joint fastened with less than the full number of bolts • the length of time the temporary joint remained in track • the train exceeding the temporary speed limit

On Sunday 27 July 2003 at about 0450, the motor tanker "Eastern Honor" grounded while entering Whangarei Harbour to berth at Marsden Point Oil terminal. The hull in the area of the bow, stern and rudder was damaged. The "Eastern Honor" subsequently berthed safely at the oil terminal where the cargo was discharged. There were no injuries and no discharge of oil into the environment. This grounding occurred about 3½ months after a similar incident involving the "Capella Voyager" (TAIC Marine Occurrence Report 03-206).

On Saturday 5 July 2003 at about 2100, the passenger freight ferry "Aratere" collided bow first with the starboard side of a fishing vessel moored at Aotea Quay in Wellington Harbour. The fishing vessel, "San Domenico", and the quay suffered extensive damage. The safety issues identified included: · the undertaking of safety critical tasks while suffering from the effects of chronic sleep loss · the adequacy of provision of medical data concerning sleep disorders in the Maritime Rules

On Friday 4 July, at 0632 the container ship "Bunga Teratai 4" and the fishing vessel "Mako" collided in Tasman Bay to the west of D'Urville Island. Neither vessel was damaged to the extent that its seaworthiness was compromised. Had the point of contact been at the bow of the ship instead of the stern, the fishing vessel would probably have been capsized and sunk with the likelihood of serious injury or death of its 4 crew. On Friday 27 June 2003, Tranz Metro Train 3347 was an Auckland to Papakura diesel multiple unit passenger train. At about 0915, a driveshaft on passenger car ADL 810 failed as the train approached Purewa Tunnel, between Meadowbank and Glen Innes stations. The DMU driver stopped the train about 200 m short of the tunnel. The free end of the failed driveshaft was not sufficiently restrained and it punctured the floor of the passenger compartment. The 3 passengers travelling in ADL 810 were not injured. The safety issues identified included:

Maritime MO-2003-208

On Thursday 16 January 2003, at about 1743, the passenger ferry "Harbour Cat" was proceeding from Birkenhead to Auckland with 3 crew and 2 passengers on board when a fire was discovered in the starboard engine space. The crew extinguished the fire and the ferry continued to the Auckland ferry terminal on its port engine alone. One of the crew suffered from slight smoke inhalation. Safety issues identified included: * access to engine rooms on passenger ferries * adequacy of procedure and training of ships’ crew in tackling engine space fires.
