On 28 February 1995, at approximately 2240 hours, Train 258, a freight from Te Rapa to Westfield passed through the area immediately north of the Waikato River rail bridge in Ngaruawahia. Circumstantial evidence indicates it was this train which collided with a pedestrian who was crossing the line outside the driveway to his house at that time. The pedestrian was killed by a train at some time after 2230 in that area. No safety deficiencies which would have prevented this accident were revealed by the investigation.

On Saturday, 1 February 1997, at about 1030, the restricted-limit passenger ferry "Te Waka o Kapanga", while en route from Waiheke Island to Rotoroa Island, struck rocks off the point between Little and Sandy Bays on Waiheke Island. None of the 53 passengers and four crew was injured but damage to the two rudders and propulsion systems was substantial.

On Sunday, 26 November 2000 at about 0105, Train 540, the northbound Longburn to Whareroa milk train, derailed near Kai Iwi while rounding a curve about 25 km/h faster than the authorised and posted curve speed of 50 km/h. Ten full milk-tanker wagons left the track and came to rest in a gully below, disgorging a large volume of milk. There were no injuries. The reason for the excessive speed in the curve was the locomotive engineer losing situational awareness during a microsleep.

On Friday 29 July 2005 at about 0506, a rake of 9 wagons that had been detached from the locomotives of Train 644 ran away to the south from Waingawa. The runaway wagons collided with a motor vehicle at Hodders Road level crossing, about 11.75 kilometres (km) from Waingawa, seriously injuring the driver. The wagons then continued for a further 2.75 km before stopping at about 70.50 km between Dalefield and Matarawa. The driver of the vehicle was taken by ambulance to Wairarapa Hospital, and subsequently air lifted to Wellington Hospital. Safety issues identified included:

On 23 December 1993 Train 404 collided with a motor vehicle near Ruakura. The safety issues identified in the investigation were the siting of trackside structures, the frangibility of trackside structures, the speed of the railcar and the timetable of the railcar service, and the conspicuity of level crossing flashing lights against the sun at low angles of elevation.

On Monday, 10 May 1999, at approximately 1825 hours, Train 211, a southbound express freight, pulled out of the Auckland Freight Centre and stopped to have a defective tail end monitor replaced. While it was stopped, a DSG shunt locomotive that was operating on a converging road ran into the rear of the train. The remote control operator who was operating the shunt from the rear refuge of the locomotive was thrown off by the impact.

Overseas assistance inquiry under Section 8(2)(e) of the Transport Accident Investigation Commission Act 1990, which authorises the Commission to "cooperate and coordinate with other accident investigation organisations overseas, including taking evidence on their behalf." The reported circumstances were that on 5 August 2025, the skipper of the Mollie, a New Zealand-registered recreational vessel, was seriously injured and later died when winch equipment malfunctioned. The accident occurred when the vessel was off the west coast of the Isle of Wight, United Kingdom.

On Tuesday, 25 June 1996, at approximately 1420 hours, a rock-fishing charter group of six passengers and one guide was returning to the beach from where they had launched in the "Hunky Dory", a 5.5m aluminium dory, when the craft was overwhelmed by seas and foundered. Five of the passengers swam for the shore. Four succeeded and raised the alarm. The fifth swimmer, after spending nine hours in the water, succumbed to exhaustion and drowned. The design, loading and corresponding freeboard of the craft combined with the sea conditions were factors contributing to the foundering.

On the second landing of a familiarisation flight in the tailwheel aircraft, the pilot under instruction flared too high. The aircraft "skipped" onto the main wheels and a series of rudder overcorrections ensued in the pilot's attempts to maintain directional control. The aircraft groundlooped to the right, the left undercarriage collapsed, and the left wingtip struck the ground.
