TAIC is investigating an accident involving South Port NZ’s pilot vessel Takitimu II. It grounded near Stirling Point, near Bluff, Southland at about 0500 on 26 December 2024. The vessel had been outbound to meet an inbound ship. The vessel reportedly sustained damage and was taken out of service for repairs. No reports of injuries or pollution.

Incident date: Publish date:At approximately 2215 hours on Tuesday 24 August 1999, northbound Train 474 struck a derailed wagon while passing southbound Train 227 between Te Rapa and Horotiu on the North Island Main Trunk line. The incident occurred when a UK wagon conveying a 40-foot container on Train 227 became derailed shortly after passing through Horotiu as a result of a tyre working loose on a wheel. The wagon continued in a derailed state, obstructing the parallel up main line as Train 474 approached.

Incident date: Publish date:Between Friday 7 May 2004 and Thursday 30 September 2004, four incidents involving traction motor fires on Tranz Metro electric multiple unit passenger services occurred on the Wellington suburban passenger network. This report incorporates reports 04-114, 04-122 and 04-124. These incidents occurred on 4 different driving sets, and all resulted in smoke entering the passenger compartments. Incident date: Publish date:On Monday 26 July 1999 at about 1250 hours, an Air New Zealand Boeing 737 was flying a circling instrument approach in the Queenstown Control Zone when a Mount Cook ATR 72 entered the zone without a clearance, infringing the separation requirements for the 2 aircraft.

Incident date: Publish date:An Air New Zealand B737 was on final approach to runway 16, at Wellington Airport when a Piper PA38 Tomahawk was cleared to cross from Wellington City to Petone, maintaining visual separation. The aircraft converged until a loss of separation occurred. Both aircraft were flying in visual meteorological conditions (VMC). The pilot of the Tomahawk attempted to take evasive action. The aircraft passed in opposite directions with about 350 m between them. None of the 68 persons on board the aircraft were injured.
