Aviation AO-2008-001

On 17 February 2008, a mid-air collision between a light aeroplane and a small helicopter over Paraparaumu resulted in the deaths of 2 student pilots (aged 17 and 19 years) and a flight examiner (aged 30). Both aircraft were destroyed and several homes and commercial premises damaged, but no persons on the ground were injured.

On Monday 9 October 2006 at 0806, passenger Train 4045 travelled 526 metres between Britomart station and Quay Park junction with the locomotive engineer not at the controls of the train. The locomotive engineer had left the train to operate a valve to restore air-brake pressure following an uninitiated emergency brake application. Once air-brake pressure had been restored, the train began to move before the locomotive engineer re-boarded the train. There was no damage and none of the 3 crew and 12 passengers on board at the time was injured. Safety issues identified included:

Rail RO-1994-104

On 9 February 1994, NZRL Train 849, a westbound unlaiden coal train on the Midland Line, encountered a digger obstructing the line on the Avoca Bank, between Staircase and Craigieburn. The crew sighted the digger in time to stop the train safely. The safety issue identified in this investigation is the adequacy of protection rules applicable to track workers.

Maritime MO-1998-214

On Tuesday, 6 October 1998, at about 0220, the Korean stern trawler "Dong Won 529" grounded on rocks at the southern end of Breaksea Islands, Stewart Island. Because of forecast bad weather, the vessel, with 39 crew on board, had been on passage from the Solander fishing grounds to more sheltered fishing grounds to the east of Stewart Island. Before the vessel could be refloated it slipped off the rocks and sank. There were no injuries. Safety issues identified included: • poor watchkeeping practices • preoccupation with non-watchkeeping duties

Addendum to 99-122: Following the publication of Railway Occurrence Report 99-122 the Commission received additional information based on a new recorded departure time of Train 919 from Clinton. This strengthened some previously expressed concerns at the interpretation which could be placed on sections of the report covering actions open to the locomotive engineer of Train 919 (LE2) on the day.

Maritime MO-1998-218

On Tuesday, 15 December 1998, at about 1510, the Ukraine registered fishing vessel "Meridian 1" was steaming off Bluff when it grounded on Anchor Ridge close outside Bluff Harbour entrance. The grounding happened when the watchkeeping officer was distracted and failed to adequately monitor the progress of the vessel. After the grounding a Southport (NZ) Limited pilot boarded "Meridian 1" and with the assistance of the 2 harbour tugs, Meridian 1 was refloated and towed into Bluff to assess the damage. No one was injured due to the grounding but "Meridian 1" suffered minor damage.

At about 1149 on Friday 14 March 2008, southbound express freight Train 923 collided with a truck on Hawke Road level crossing, Orari, between Rangitata and Temuka on the Main South Line, when the truck moved into the path of the approaching train. The level crossing had passive control "Stop" signage. All signage and road markings were in place and in good condition.

Aviation AO-1999-006

On Saturday 18 December 1999 at about 1524 hours, ZK-HYE, a Hughes 269C helicopter, was on a private local scenic flight about 3 nautical miles north of Kawerau. Approaching to land, the helicopter struck a power line and impacted the ground nearly inverted. The pilot and 2 passengers died as a result of the accident and the resulting fire destroyed most of the helicopter. No new safety issues were identified during the investigation.

The Republic of Kiribati established a Commission of Inquiry into the capsize and foundering of the single outrigger passenger ferry Uean Te Raoi II off Maiana Island on 13 July 2009, which resulted in 35 deaths and 16 survivors. The New Zealand Transport Accident Investigation Commission was asked to provide within one month an interim report to the Kiribati Commission to enable the Kiribati Commission to report back to the Kiribati Government within the deadline described in its terms of reference.

Aviation AO-2002-002

On Friday, 25 January 2002, at about 1430, Piper PA34-200T Seneca ZK-SFC was on approach to land at Gisborne Aerodrome when the nose undercarriage failed to extend. After several unsuccessful attempts to extend the nose undercarriage, the pilot diverted to Hastings Aerodrome where a full wheels-up landing was completed. The 2 crew members and one passenger on board were uninjured and the aircraft sustained minor damage.

On Wednesday 11 March 1999, at approximately 1120 hours, a head-on collision occurred between the middle ferry shunt and the siding shunt at Wellington ferry terminal. The locomotive engineer's seat in the siding shunt locomotive was torn from its mountings, and the locomotive engineer was propelled to the front of the cab, sustaining lacerations and concussion. Two shunters riding on the leading ends of their respective shunts were able to jump clear before the collision. The safety deficiencies identified included:
