In the early hours of Wednesday 27 October 2004, the San Rochelle with a Master and 2 crew on board was in the fishing grounds to the north of New Zealand. At about 0345 with the crew part way through setting the first longline of the voyage, the Master noticed smoke coming from the aft engine room ventilators. He discharged a portable CO2 fire extinguisher into the engine room, without the fire noticeably diminishing, before he made a distress call on his single side band radio. The distress call was acknowledged by Maritime Radio and a rescue operation commenced.
Incident date: Publish date:On Wednesday 19 September 2001, at about 0705, a collision occurred between a light locomotive and a private motorcar at Egmont Tanneries Ltd private level crossing between Stratford and Inglewood on the Marton to New Plymouth Line. The motor vehicle had made a right-hand turn from State Highway 3 which ran parallel to the railway line and entered the unprotected level crossing immediately in front of the approaching locomotive.

Incident date: Publish date:At about 0530 on Saturday 18 September 2010, the coastal container ship Spirit of Resolution departed its berth at the port of Onehunga in Manukau Harbour and took about 1.7 hours to reach the Manukau Bar at the harbour entrance. The sea conditions were forecast to exceed the four-metre maximum permissible observed wave height over the Bar and remain so for at least another two days. The master decided to proceed to the harbour entrance, knowing that he had the option of anchoring inside the entrance and awaiting more favourable conditions.

The reported circumstances were that the train was travelling towards Christchurch. The train had passed through Otira tunnel when the locomotive engineer reported diesel leaking onto the running board of the lead locomotive.

Incident date: Publish date:At about 1345 hours on Saturday, 26 June 1999, a vintage steam train operated by the Bay of Islands Vintage Railway was on a scheduled passenger trip from Opua to Kawakawa when the track spread and the locomotive and the following two carriages derailed at low speed. No injuries to the crew or passengers resulted. Safety issues identified included the standard of track maintenance and the adequacy of the track inspection. Two safety recommendations were made to the operator, and two to the Director of the Land Transport Safety Authority to address the safety issues.
Incident date: Publish date:On Tuesday, 1 October 1996, at approximately 1930 hours, the longline fishing vessel "Amigo", while anchored off the Waitaha River mouth, approximately 17 miles south-west of Hokitika, was overcome by seas and foundered. Searchers located wreckage from the "Amigo" on the beach the following morning. The body of one deck-hand was recovered some days later. The Master and another deck-hand were presumed drowned. The cause of the accident could not be identified conclusively.
Incident date: Publish date:On Wednesday 16 April 2003, at about 1820, the motor tanker "Capella Voyager" grounded while entering Whangarei Harbour to berth at Marsden Point Oil terminal. The hull in the area of the forepeak tank was set up, creased and ruptured, water flooded into the forepeak tank. The "Capella Voyager" subsequently berthed safely at the oil terminal where the cargo was discharged. Temporary repairs were effected and the forepeak pumped out at Marsden Point before the ship sailed for Singapore to complete permanent repairs. There was no discharge of oil into the environment.
Incident date: Publish date:On Sunday 28 March 2004, at about 1541 shortly after entering the Otira Tunnel Train 804, the Greymouth to Christchurch Tranz Alpine passenger express, with 268 passengers and 6 crew on board, stalled and slid backwards. The locomotive engineer eventually brought the train under control but when he attempted to move forward again the locomotives could not develop traction and the train stopped.

Incident date: Publish date:The reported circumstances were that the Niue flagged self-propelled barge with 11 crew on board, had been anchored outside Westport Harbour. In poor weather, it dragged anchors and grounded on nearby Carters Beach at about 1145pm on 31 Aug 2024. A protection order preserves and protects evidence - including all electronic data from the vessel and onboard equipment such as propulsion, power supply and anchor systems. Nobody may access the items listed unless permitted by the Commission. The master and others can still act for the safety of the vessel, environment or people on board.
