Rail RO-2008-101

At about 1149 on Friday 14 March 2008, southbound express freight Train 923 collided with a truck on Hawke Road level crossing, Orari, between Rangitata and Temuka on the Main South Line, when the truck moved into the path of the approaching train. The level crossing had passive control "Stop" signage. All signage and road markings were in place and in good condition.

On Saturday 18 December 1999 at about 1524 hours, ZK-HYE, a Hughes 269C helicopter, was on a private local scenic flight about 3 nautical miles north of Kawerau. Approaching to land, the helicopter struck a power line and impacted the ground nearly inverted. The pilot and 2 passengers died as a result of the accident and the resulting fire destroyed most of the helicopter. No new safety issues were identified during the investigation.

The Republic of Kiribati established a Commission of Inquiry into the capsize and foundering of the single outrigger passenger ferry Uean Te Raoi II off Maiana Island on 13 July 2009, which resulted in 35 deaths and 16 survivors. The New Zealand Transport Accident Investigation Commission was asked to provide within one month an interim report to the Kiribati Commission to enable the Kiribati Commission to report back to the Kiribati Government within the deadline described in its terms of reference.

On Friday, 25 January 2002, at about 1430, Piper PA34-200T Seneca ZK-SFC was on approach to land at Gisborne Aerodrome when the nose undercarriage failed to extend. After several unsuccessful attempts to extend the nose undercarriage, the pilot diverted to Hastings Aerodrome where a full wheels-up landing was completed. The 2 crew members and one passenger on board were uninjured and the aircraft sustained minor damage.

On Wednesday 11 March 1999, at approximately 1120 hours, a head-on collision occurred between the middle ferry shunt and the siding shunt at Wellington ferry terminal. The locomotive engineer's seat in the siding shunt locomotive was torn from its mountings, and the locomotive engineer was propelled to the front of the cab, sustaining lacerations and concussion. Two shunters riding on the leading ends of their respective shunts were able to jump clear before the collision. The safety deficiencies identified included:

Maritime MO-2000-205

At about 0914 on Wednesday, 31 May 2000, the passenger ferries "Quickcat" and "Quickcat II" were operating on the ferry service between Waiheke Island and Auckland when they collided about 0.5 miles east of the northern leading light in Auckland Harbour. The visibility in the area at the time of the collision was about 50 m due to fog. There was a total of 127 passengers and 7 crew aboard the 2 vessels, none of whom were injured. Safety issues identified included: • the speed of the 2 vessels in restricted visibility • the inefficient use of radar for collision avoidance

Rail RO-2005-115

On Friday 1 April 2005, Train 2100 was a scheduled empty passenger service being positioned to Waitakere to start passenger operations for the day. At 0627, soon after the train had passed through Ranui station, the coupling between the locomotive and the guard’s van parted. As a result of the parting, the automatic brake system lost all air pressure and brought the train to a stop.
